Social Care and NHS services all over the country, are working on local plans for "Integrated Care" where budgets are pooled, and the patient is at the centre of the service.
Will this result in a person centre, joined up care for the patient? A seamless service across agencies? Will patients gain control over a single budget pot, and be able to determine their own care?
Or is this a way of bringing in efficiency savings where patients make the cuts themselves? And will it actually work?
We have invited those involved in plans to integrate care in Hackney to come to listen and respond to local people and community organisations about their ideas for person centred care, and whether current plans will achieve this.
