How does integration relate to the One Hackney & City pilot?

The One Hackney & City pilot was designed to ensure vulnerable patients received better and more integrated care. It involved GPs referring residents with high care needs to an integrated team of clinical, community, voluntary and social care workers. These teams supported residents to remain healthy and independent in four geographic areas – called ‘quadrants’ – in Hackney and City.

The pilot was funded on a one-off basis through the Better Care Fund (BCF) for joint local authority and NHS integrated care. It ran from April 2014 to March 2017. Healthwatch Hackney has produced an evaluation report here and video interviews with some of the patients here and below.

The Unplanned Care workstream is looking at how to build on the successes and lessons learned from the One Hackney & City pilot by developing a neighbourhood model of integrated care.
